I always thought he still had stitches in his cheek when Sahid and Desmond first met him. It could be then that the thread marks on his cheek look more apparent now that he's bloated with water.

However, there wouldn't be much sense to him having stitches if they weren't going to mean something with the purposeful anachronisms that keep popping up.
