So, sometime after Charlie and Desmond got the code, Mikhail pulled the spear out of his chest, went over to the far wall where the breathing gear was, put on a set of tanks and face mask, went back to the moon pool (bleeding the whole way) and hopped into the water without making any noise that Des or Charlie could hear.

And weren't Charlie and Des between Mikhail and the scuba gear while they were talking to the dying woman about the codes?

I don't think Mikhail had the whole oxygen tank on...I think he just had the goggles on.

Yeah, it seemed like he only had on the scuba mask, no respirator or anything.

Mikhail was pretty upset when he found out there was jamming equipment. Maybe he did not know the equipment was waterproof, and so as Mikhail's last hurrah he decided not only to kill charlie but to destroy the jamming eqip. also.
