I know that Jack took some pills because he was injured in the car crash on the bridge. But do you suppose that he started drinking because he felt "out of sync" with time in the real world, his body had become used to island time? Maybe the Kate and everyone else adjusted better than Jack.

forced to do an operation
mercy killed Libby

saw death of many

combat medics often have undiagnosed PTSD
and Jack may be self medicating for many reasons including guilt powerless to help others...

When was Jack tortured? (Also he tried really hard to save Boone and the marshall who had captured Kate, so that MUST have compounded the PTSD.
