In the scene just before he gets up, it is apparent that he is also paralyzed again (he has to move his legs with his hands before he can reach the pistol in the dead guy's holster).
Then Jacob (appearing under the guise of Walt) tells him to "Get up. We have work to do."
Locke lays back and then smiles.
I think the island (or the Jacob/Walt apparition) performed another miracle on him.
