I lean toward what Logan wrote - it seems to explain the different photos of Desmond and Penny. And maybe how Desmond could have gone to medical school and almost finish (but had to leave to take care of his 3 brothers), made sets for the Royal Shakespeare company, served in the military, dated a woman for 6 years, trained for a race around the world, lived as a monk, dated Penny for two years, lived on the island for 3 years - and even though we don't know for sure how old Desmond is, he can't be that old, twenties? - so CableRunner and Logan's multiverse might explain how Desmond did all of this in his lifetime, especially since he isn't that old.

Sounds like a couple of friends I know. They travel around the world jumping form job to job and major in numerous fields never committing to one. They do so because they can't bear the thought of responsibility so they run from it thinking it will eventually stop chasing them. The things Desmond has done in his early life don't seem all that unbelievable to me but again I have met very very weird individuals throughout my short life time so there’s no telling what’s out there.
Maybe your right there could be a set of different realms and that theory does answer a lot of questions as to what’s going on with Des and the island. If this ends up being true I wouldn't be surprised at all but I hope this isn't the case. I am really looking for a more sophisticated scientific phenomenon to answer the questions regarding the island and the Losties. Not saying dual time lines isn't sophisticated nor scientific but with all the knowledge the others posses about the island and the D.I, dual timelines just don't mix in well with how the "others" operate. It strips the notion of free will and the ability to choose what one does.
If you are able to change something you have done in the pass because some how you were able to collide with yourself in another realm or time travel then “choice” is no longer important. I am sure all of us at one point wished we could have changed something in our pass and if given the opportunity almost all of us would. That in it self is why it can’t happen. The point of life is accepting you have one shot at it during one life-time at a time and in that life-time you can only move forward not backwards so giving that information we all must make decisions knowing consequences will follow. Going back in time or colliding in another realm and changing your wrongs to rights gives no purpose to life. (Past, Present and Future). A person would be able to eliminate “wrong” therefore also eliminating “right”. For if “wrong” never existed neither would “right”
Now maybe in the after life or if you believe in reincarnation this is something that could come into effect. Being able to some how remember the wrong things you did in your previous journey and being able to prevent yourself from making the wrong choice again.(Which seems like a logical meaning to life) Maybe Des had a vision or déjà vu from one of his previous life-times and intertwined/connected it with the present one. Maybe he didn’t see what we saw in his flashbacks; maybe that’s how his brain interpreted it. Let’s say for argument sake reincarnation existed. I mean if any of us had a flash back or remembrance of one of are previous life times like Des did how would our brain interpret the occurrence? Déjà vu weird feeling you have been there before but during your present life-time you haven’t? Hallucination? Coma? Body goes into shock? Memory lost? Or would we be able to remember we had a previous life and we were some one else at one point?
