There is really no good reason, my theories:

- something to do with Eko's state of mind in each case. Maybe because he was more calm/resigned to the experience the first time, and more aggressive the second. His point of view right before he died seems to me to be a bit more selfish than it was when he still thought of himself as just a priest.

- that Smokey has several "handlers" and that they direct what it does and where it goes...and that Eko's threat to these individuals was increasing so he was taken out. When I suggested to my wife that the handler who spared him was Walt, she thought I might be onto something. Or on something...

- that the energy he was exposed to in the Hatch explosion somehow made him appear in a different light, so to speak...marked him for death in a way that we can't understand yet.

- or Smokey was just having a bad day and took it out on the people that were closest to it.
