Well Radzinsky was that brown spot on the ceiling, but now that the hatch is gone there is no sign of his existance..Unless he faked his death to Kelvin, Kelvin ofcourse died when Desmond body-slammed him in to some rocks.

So I think the eye patch man could possibly be Radzinsky.
